Our primordial concern is to provide you with the necessary, appropriate, safe and quality health care based on the assessment and requirements of your primary care physician. We help you achieve independent living in the comfort of your residence, while considering the limits and demands of your medical conditions. We recognize the innate differences among individuals and we design our plan of care according to your unique needs, while respecting your privacy, property and dignity.
When you are able to perform your own care as observed by our home health team or when you have reached your optimum level of recovery and no longer require skilled care as certified by your physician, we shall discontinue our services. You will receive advance notice and a pre-established discharge plan for you to follow.
